ALS Caregiver Job Description Example
Job Description: Caregiver for a XX-year-old client , living with AMYOTROPHIC LATERAL SCLEROSIS (ALS, Lou Gehrig disease). The client has a tracheostomy and requires mechanical ventilation 24/7. The client has minimal movement and communicates via an eye-gaze communication device. The client splits time between his bed and his power wheelchair.

Monday - Friday 6pm-8pm ##
Saturday Sunday 4 Hour Shifts, typically 10am-2pm ##
Monday Wednesday Friday 8am-12am ***
Tuesday Thursday 8am-11:30am ***
Monday Wednesday Friday 11am-4pm ***
Tuesday Thursday 11:30am-5pm ***

RESPONSIBILITIES:
Follow the written client service plan, which may include the following services:
Companion Services - Limited assistance and lite hands-on care.
Provide companionship, safety, and supervision for the client.
Homemaker Services - Assist with daily activities (lite hands-on care). Includes
Companion Care.
Tasks may include:
General housekeeping and cleaning of client’s home, including vacuuming, dusting, cleaning bathrooms, and dishes, sweeping and mopping floors, making beds, dusting, emptying trash, pet maintenance, and all other tasks indicated on the service plan.
Laundry, including washing, drying, folding, and putting away clothes.
Gravity tube feedings with tube feed formula. Connect and disconnect feeding tube, administrator tube feedings and medications, flush G-tube, clean and maintain G-tube and supplies.
Tracheostomy and ventilation circuit care maintenance. Clean tracheostomy, circuit tubing, and accessories, maintain ventilation humidifier, and power supply, clean and maintain suction tubing, machines, and yonkers.
Companionship, including activities, discussions, encouragement, and reassurance as needed.
Personal Care Services - Non-medical, hands-on assistance with activities of daily living.
Tasks may include:
Assist clients with activities of daily living (ADLs), such as bathing, toileting, dressing, and personal hygiene.
Ambulation and transfer assistance.
Medication preparation and medication assistance, pending evaluation: To assist clients with maintaining medication schedules and support to access medication as needed.
Assist with eye and skin care, the including the application of over-the-counter, non-medicated ointments or lotions. "Body care", including nail clipping.
Safety checks around the house (security, dangerous situations, etc.)
Redirect clients; provide safety guidance protection to the client who cannot be left alone due to impaired judgment.
Bathing Visit (1 hour): To assist with bathing, including (a) guiding or supervising the client bathing, and (b) assisting the client with difficult tasks such as getting in or out of the tub or washing the back.
Non-weight bearing transfers.
Additional duties:
Maintain excellent and objective ongoing communication with spouse and staff; respond to phone calls and texts promptly.
Document thorough care provided on the Client Service Record, including initials and signatures.
Clock in consistently and on time through the online app system, using employees' phones.
Time off requests should be submitted at least two weeks before the request, so it can be approved.
Provide updated and accurate information, documents, licenses, etc., to maintain personnel folders and requirements for field placement.
Work the scheduled shifts assigned in a timely and consistent manner.
Notify the client when availability changes two weeks in advance.
Maintain a professional relationship with the client and family.
Maintain confidentiality at all times.
Assure all services and operations comply with the STATE YOU LIVE Revised Statutes and the West Virginia Administrative Rules governing home care agencies. Additionally, comply with the Administrative Rules governing the STATE YOU LIVE Aging and Disabled Waiver Program.
Implement all tasks as instructed to support compliance with OARs.
Qualifications:
Must have first-aid and CPR certification from accredited sources.
Must live local to LOCATION, and have reliable transportation.
Verifiable and or training in providing care for disabled persons.
Demonstrated competency in the skills required to meet the needs of our clients.
Ability to lift 75 lbs. (required for most clients)
Ability to read and communicate effectively in English.
A current personal phone or cell phone where we can contact you.
Reliable transportation to and from the client's home in LOCATION.
An accurate employment application and two positive work-related references.
A driver’s license in good standing, decent driving record (less than 3 moving violations in the last 3 years and no serious violations), basic auto insurance, and a reliable car.
An acceptable criminal background check in every state you have lived or been employed.
Be comfortable around clients, children, and dogs. (Children are XX years old, and the dog is a friendly lab)
Feed, water, and let the dog outside to use the potty. He likes treats.
Preferably an employee is a non-smoker.
Meet all other company employment criteria.
